If you need support to live your daily life, then you may be entitled to support from the Council under Self Directed Support .

As someone who is eligible for Self Directed Support, you will get a Personal Budget to buy the support of your choosing. In order to access this money and to tell the Council how you are going to spend it you will need to plan and write a Support Plan.

What is a Support Plan?

A Support Plan is a document which describes to the council how you are going to spend your money. It lays out an action plan for you, to help your support and live your life. It also tells service providers how you would like services provided.

You may choose to manage your support entirely by yourself, or ask others, such as your carer, family or friends to help you. You may also choose to have your support managed by us or by another organisation on your behalf.

What must you think about whilst doing a support plan?

1. What is important to you?
2. What do you want to achieve or change
3. How do you want to be supported?
4. How will you use your budget?
5. How will your support be managed?
6. How will you stay in control of your own life?
7. What are you going to do to make this plan happen?

Your support plan will be reviewed annually or at more regular intervals if necessary to make sure it is meeting your needs.

Eligibility

If you are aged 18 or over and are eligible for social care support you may have this support provided through a Personal Budget. (Adult Mental Health Social Care Services provides support to people over the age of 16). There are some exceptions to this, such as if you need residential care, emergency support or are subject to continuing health care or some statutory mental health orders. Personal Budgets for carers may be offered in the future.

What you need to do

If you already receive support for your social care needs, at review, your social care worker will give you support in transferring your services to Self Directed Support.

If you are enquiring about social care support for the first time you should contact the council and ask for an assessment to check your eligibility for services. You can do this at any time.
