Community Pathway

The Lancashire Community Pathway is a hub of Lancashire wide local voluntary/community/faith and user led organisations who are looking to provide support plans and managed accounts to anyone with a personal social care budget.

This means if you need help to write a support plan or manage your budget you can use the pathway to identify an organisation that will help you.

First you have to be eligible to have a personal budget. If you think you are eligible contact Social Services on the hub on 0845 053 0009 and ask for an assessment.

Once you have had an assessment you can ask your social worker to refer you to the Lancashire Community Pathway.

A Community Pathway organisation will then be in touch to arrange an initial meeting to introduce themselves and discuss your requirements.

They will work with you to help you to develop and create your own support plan and can offer advice and information on local resources that may be of use to you in your community.

Once your support plan is agreed by Lancashire County Council the organisation you worked with can also help you to implement your support.

We have a number of organisations that work with disabled people, mental health service users, older people, people with learning disabilities and carers.
